At Horsham Dental Care, we make every effort to accommodate pediatric dental emergencies ASAP because we understand both the urgency and the anxiety that can come with seeing your child in pain.

Common pediatric dental emergencies and our approach:

  • Toothache: If your child wakes up with a throbbing toothache, it could be a deep cavity or even an abscess. We’ll want to see them quickly. In the meantime, gently cleaning around the tooth and a child-appropriate dose of pain reliever can help. Once here, Dr. Mathews or Dr. Lala will examine and probably take an X-ray. We treat the cause: if it’s a cavity, perhaps a nerve treatment (pulpotomy) and a filling/crown; if an infection has spread, maybe an extraction if the tooth isn’t savable. We always attempt to save baby teeth if possible (they hold space for permanent teeth), but if removal is needed, we can discuss a space maintainer. We ensure the child is numb and comfortable – often using “sleepy juice” (local anesthetic) and maybe nitrous oxide which works wonders to relax them.
  • Dental Trauma: Kids might fall off a bike, collide during sports, or take an unfortunate tumble, and teeth can get injured. If a baby tooth is knocked out, do not replant it (risk of damaging the adult tooth). Just focus on stopping bleeding (bite on gauze) and see us to ensure no fragments remain and that the area is okay. If a permanent tooth is knocked out (avulsed), time is critical: gently rinse it (no scrubbing), keep it moist (milk is best as described here at mouthhealthy.org, or in the child’s mouth between cheek and gums, or even saliva) and get to us within 30-60 minutes. We will attempt to replant it. If a tooth is chipped or cracked, keep any large pieces in milk and bring them. We can often bond the piece back or use filling material to restore it. If the tooth is pushed out of position (luxated), we might reposition and splint it. For any facial trauma, we also check for lip or tongue lacerations – minor ones we can manage, more serious might need stitches. We keep the atmosphere calm and even a bit fun (sometimes telling brave stories or distractions) so the child doesn’t panic.
  • Orthodontic or Appliance Issues: If your child has braces and a wire is poking, we can clip it or wax it. If a spacer or space maintainer comes loose, we’ll address it. If a retainer is broken, we’ll advise next steps.
  • Abscess or Swelling: A gum boil or facial swelling in a child usually indicates an infected tooth. We treat similar to adult – antibiotics if needed to stabilize, then likely a “baby root canal” or extraction of the baby tooth to remove the source. Untreated infections can spread quickly in kids, so never wait on swelling – call us right away.
  • Object Stuck in Teeth: Kids can get all sorts of things caught (from popcorn hulls to random objects). If floss can’t get it out, we will. Also, if a kid puts something in their mouth and it’s stuck (like their finger in a floss container? you’d be surprised), we can help or direct appropriately.

Our emergency protocol: We prioritize kids in pain. If it’s during office hours, we strive to have them seen the same day. If an emergency happens after hours, our phone will have instructions – one of our doctors can often be reached for guidance. If it’s a major injury involving head trauma or uncontrolled bleeding, go to the ER first.

We always aim to alleviate pain quickly and then do any definitive treatment in a child-friendly manner. We might use euphemisms (the drill is Mr. Whistle, etc.), and we stop if the child is too distressed – sometimes we reschedule with sedation if needed. But many times, kids surprise their worried parents with how well they handle an emergency visit – especially when they see familiar, friendly faces at our Horsham office rather than strangers.
