What is a Root Canal (and Why Might You Need One)?

Inside every tooth is a soft tissue called the pulp, which contains nerves and blood vessels. If the pulp becomes infected or irreversibly inflamed – due to a deep cavity, crack, or trauma – it can cause intense pain and lead to an abscess (infection) in the jaw. Root canal therapy is the process of removing that infected pulp tissue, cleaning and disinfecting the inner canals of the tooth, and then sealing them to prevent future infection.   • Isolation: We place a small protective sheet called a rubber dam around the tooth. This keeps the area dry (saliva-free) and prevents any debris or disinfectants from getting in your mouth. It also helps you feel safer, since nothing will accidentally fall to the back of your throat.
  • Cleaning the Canals: The doctor will create a tiny opening in the top of the tooth to access the pulp chamber. Using specialized small instruments, she will carefully remove the infected pulp from inside the tooth and root canals. We use rotary endodontic tools (electric-powered files) that are efficient and quiet, shortening treatment time compared to older hand files. You’ll mostly just hear a soft hum. Throughout, we irrigate (rinse) the canals with a disinfectant solution to flush out bacteria and debris.
  • Filling the Canals: Once the canals are thoroughly cleaned and shaped to the proper size, we fill them with a biocompatible material called gutta-percha. This rubbery material is placed into the now-empty canals along with adhesive cement to completely seal them. Sealing the canals is crucial to prevent re-infection. The access hole we made in the tooth is then filled with a temporary filling material. At this point, the nerve is gone, the infection is gone, and you should feel much better – often patients who had pain feel immediate relief as soon as the nerve is removed!

A root canal on a back tooth can take about 60-90 minutes, front teeth often less. If the tooth has multiple canals (molars usually have 3-4), it may take a bit longer. We’ll let you know what to expect time-wise. If needed, we can do the treatment in steps (remove pulp and medicate tooth, then finish at a second visit) – but usually it’s best to complete it in one visit.

After the Root Canal – Getting a Crown

After root canal therapy, your tooth will be free of infection, but it will also be more brittle (since the living tissue and blood supply is gone). That’s why in most cases we recommend a crown after a root canal, especially for molars and premolars. A crown (cap) will protect the tooth from fracture and restore it to full strength for chewing.

Here’s the typical follow-up: about 1–2 weeks after your root canal, once any tenderness subsides, you’ll return for a crown prep. Dr. Lala will remove the temporary filling and any weakened tooth structure, then shape the tooth and take impressions for a crown. A custom crown (usually porcelain or porcelain-fused-to-metal) will be made to fit over the tooth. We’ll place a temporary crown in the meantime. Finally, the permanent crown is cemented on, and your tooth is as good as new – without the pain! If the root canal was on a front tooth, sometimes a filling may suffice if enough tooth is intact, or an aesthetically pleasing crown can be placed – we’ll discuss the best restoration for your situation.

Does a Root Canal Hurt?

This is the #1 question people have. The great news is: no, a root canal should not hurt – it actually relieves pain. With modern anesthesia, the procedure itself is comfortable. After the treatment, you may have some mild soreness or jaw tenderness (from holding your mouth open) for a day or two. This is usually managed with over-the-counter pain relievers like ibuprofen. We will provide instructions for post-treatment care, which include being gentle on the tooth until the crown is placed and reporting any severe pain (which is rare).
